Culture In Motion strongly recommends that you consult with your physician before beginning any exercise or dance program. You should be in good physical condition and be able to participate in the exercise. Culture In Motion is not a licensed medical care provider and represents that it has no expertise in diagnosing, examining, or treating medical conditions of any kind, or in determining the effect of any specific exercise on a medical condition. You should understand that when participating in any exercise or dance program, there is the possibility of physical injury. PHOTO RELEASE: I authorize Culture In Motion Dance Studio to use all photographs or videos taken of me or my child during class, performances & events for advertising, instructional, and/or promotional material. MEDICAL RELEASE AND ASSUMPTION OF RISK: As the parent, agency/representative, or legal guardian, I hereby give consent for CULTURE IN MOTION DANCE STUDIO to provide all emergency medical/ dental care prescribed by a duly licensed physician (MD) or dentist (DDS) for my child/relative, under whatever conditions are necessary to preserve the life, limb, or well being of me or my dependent. It is the responsibility of every individual, his/her parents, or a legal guardian to provide his/her own accident or health coverage while participating in all CULTURE IN MOTION DANCE STUDIO activities. CULTURE IN MOTION DANCE STUDIO does not provide any accident or health coverage for its members or guests. By participating in CULTURE IN MOTION DANCE STUDIO activities and programs, I agree to assume full responsibility for the risk of such activities and programs and, further, agree to release and hold harmless CULTURE IN MOTION DANCE STUDIO and its staff members from any and all claims, suits, losses or related injury or death, accident or otherwise, during or arising in any way from the activities or programs of CULTURE IN MOTION DANCE STUDIO. I acknowledge and agree that this general release of liability of CULTURE IN MOTION DANCE STUDIO is binding upon me, personally, as well as in capacity of the parent or guardian of my child and on my heirs, personal representatives, successors and assigns. CULTURE IN MOTION DANCE STUDIO cannot accept responsibility for your child except when he/she is under the supervision of an appropriate employee. Parents will be responsible for notifying staff of a child’s absence, late arrival, early or late pick-up, and any other special circumstances.
